2025 Swine EID Order Form

Swine EID Tag Order Process
1. Fill out the order form below - provided address will be considered the 'shipping address'
2. Complete payment for selected quantity of tags
3. Once receipt of order and payment has been verified by Aksarben Staff the tags will be shipped.

Cost:
- $18.00 per tag plus shipping fees. This applies to all states eligible for Aksarben.
- Provide a valid email address and phone number, as it will be used to contact you in regards to your order, as needed.
- Swine EID Tags are nonrefundable.
